The Journey to Self: Top Surgery for FTM

Today, I want to share an inspiring patient’s journey of top surgery for FTM (female-to-male) transition. This procedure is not just about altering physical appearance; for my patient, it’s a significant step in aligning their external identity with their internal truth.

The Surgical Approach

In this particular case, my Beverly Hills patient required a double mastectomy with free nipple and areola grafts. This approach was chosen not solely due to the size of the breasts but also because of the excess skin, the low position of the nipple-areolas, and the substantial amount of breast tissue.

Unique Aspects of the Surgery

What made this surgery unique was the separate grafting of the areola and the nipple. The patient’s original nipples were too large for a masculine chest, so I decided to graft a smaller portion of the areola and the nipple to achieve a more masculine appearance. These surgeries are my favorite, not only because of the technical challenges they present but because of their profound impact on my patients’ lives.

During the procedure I meticulously removed the areolas and nipples, followed by the breast tissue, ensuring the preservation of the chest’s natural anatomy. The patient had chest tattoos, which added another layer of complexity to the procedure. While aesthetics are important, the correct placement of the areola took precedence.

No More Constraints

After the surgery, the patient’s chest had a more masculine contour, with symmetric incisions and well-placed areola and nipple grafts. The result was a chest that looked natural and freed the patient from the discomfort and constraints of binding. This surgery is a celebration of identity and a step towards living authentically.

Top surgery for FTM individuals is more than a physical transformation; it’s a crucial part of the journey towards self-acceptance and comfort. As a surgeon, being a part of this process is incredibly fulfilling. It’s about giving individuals the freedom to be proud of their bodies and live their truth without constraints.
