FTM TOP Surgery Techniques

As a board-certified plastic surgeon specializing in FTM Top surgery, one of the discussions I have with patients is what the most appropriate surgical procedure is. The best technique is based on two things, breast size and nipple position.
Those with larger breasts who have lower positioned nipples benefit from the Double Incision technique with Free Nipple Grafts. This technique allows for the correction of the excess tissue, the excess skin, as well as the proper positioning of the new smaller nipple and areola.
Those with small breasts, good skin tone, and proper areola position would benefit from the keyhole technique. This involves an incision on the lower half of the areola whereby the tissue is removed. Liposuction for chest contouring can also be performed through the same incision. If the areola also needs to be reduced, typically this is done in a later stage to decrease the risk of jeopardizing the blood supply to the nipple and areola. To determine the best technique, of course requires an in person physical examination to not only assess the size of the breasts but also the skin tone and elasticity.
