Male Body Contouring After Weight Loss

One of the most rewarding aspects of being a plastic surgeon is helping patients who have achieved incredible personal goals. In today’s case, I had the opportunity to perform body contouring after major weight loss for a young man who had lost over 150 pounds. He worked incredibly hard to drop from his heaviest weight of 300 pounds to around 160. This was life-changing for him, but it also left him with significant excess skin, particularly around his chest and sides. This is where my expertise and skills came in.

The Patient’s Condition

This patient’s case wasn’t a typical gynecomastia surgery — it was chest and body contouring after major weight loss. He had undergone a tummy tuck a few years ago at a different practice, but it didn’t quite address all his needs. He still had loose skin on the sides of his abdomen that extended around to his back. His chest also had significant deflation, with low-hanging nipples and loose skin around the areolas, all results of the drastic weight loss.

When patients lose this much weight, their skin often doesn’t retract as much as they would like. That’s why this surgery is so important. It helps remove the excess skin and tighten the areas that have been left behind after weight loss.

My Surgical Plan

For this patient, I focused on several key areas:

Chest Contouring
His chest was completely deflated, with loose skin that needed tightening. The nipples were also very low and needed to be lifted. I decided to remove the extra skin, raise the nipples to a more natural position, and reduce the size of the areolas to fit the new chest proportions.

Extended Skin Removal
His previous tummy tuck hadn’t fully addressed the sides and back, which still had significant excess skin. I extended the incision around his back to ensure a smoother, more natural contour. This approach prevented the “dog-ear” effect, where skin bunches up at the sides due to incomplete removal.

Tightening and Smoothing
After removing the skin, I focused on ensuring the chest had a more defined and smooth contour. This helps highlight the muscle structure buried under the loose skin.

The Final Result

After the procedure, the patient’s chest looked much more proportionate to his body, see here on my YouTube channel. The body contouring after weight loss smoothed out the excess skin around his chest, sides, and back. His pecs were much more defined, his areolas were smaller and in a better position, and the overall symmetry of his chest was dramatically improved. The incisions will heal well and, in time, will be almost unnoticeable.
