Gynecomastia and the Asymmetric Chest

Gynecomastia comes in all shapes and sizes.  When examining a patient with gynecomastia, I take time to look at any natural asymmetry (or difference) between the left and right chest. There are many different ways that the chest can be asymmetric.

  1. Volume asymmetry: There is a larger amount of glandular or fatty tissue on one side than the other. Also, the location of the tissue may be different with one side having more tissue in a certain part of the chest than the other side.
  2. Areolar asymmetry: One areola is lower than the other. Also, one areola may be larger than the other but this is not atypical in the larger side as there is more tissue that pushes out against the areola.
  3. Skin asymmetry: In severe cases there may be more skin or skin laxity on one side than the other.
  4. Nipple asymmetry: One nipple may be more or less protruding than the other

All of these factors need to be taken into consideration in order to get the best, most symmetric result for the patient.

In the photo below you can see that this patient presents with left sided gynecomastia only. In this case the surgery was performed only on the left side in order to match the right side.
